You can join our dental savings plan online through our illumitrac smile club signup form. When you join our membership plan you take care of your preventive care with a subscription for an entire year. This will save you money, particularly if you plan your procedures in advance, ensuring you can receive the care you already need at discounted rates.
Adults Membership
13 and Older
$30.30 / Month + $99 Lifetime Activation Fee
- Professional dental cleanings (2 per year)
- Doctor exams (1-2 per year)
- Needed x-rays*
- 1 emergency exam per year (if needed)
- 15% off all other treatments (some exclusions may apply)
- $99 Lifetime Activation Fee includes the first month
For patients with regular cleanings and absence of periodontal disease
Perio Membership
Periodontal Disease
$50.30 / Month + $99 Lifetime Activation Fee
- Periodontal maintenance cleanings (3-4 per year)
- Doctor exams (1-2 per year)
- Needed x-rays*
- 1 emergency exam per year (if needed)
- 15% off all other treatments (some exclusions may apply)
- $99 Lifetime Activation Fee includes the first month
For patients enrolled in active periodontal maintenance due to prior treatment of periodontal (gum) disease.
Child Membership
12 and Younger
$25.30 / Month + $50 Lifetime Activation Fee
- Professional dental cleanings (2 per year)
- Doctor exams (1-2 per year)
- Needed x-rays*
- 1 emergency exam per year (if needed)
- Fluoride treatment
- 15% off all other treatments (some exclusions may apply)
